At San Jose Behavioral Health, we welcome the opportunity to form productive relationships with professionals who refer patients to our program. We encourage mental health clinicians, hospital emergency department personnel, community agencies, physicians, court systems, and any other sources who work with individuals who can benefit from our services to contact us to discuss how we can work together for the betterment of individuals in crisis.
Our program provides comprehensive inpatient services to adults and adolescents who are have been struggling with a variety of emotional and behavioral issues, including but not limited to the following:
- Adjustment disorders
- Aggression
- Anxiety disorders
- Bipolar disorder
- Depressive disorders
- Homicidal Ideation
- Posttraumatic stress disorder
- Psychosis
- Suicidal Ideation
- Schizophrenia
- Schizoaffective disorder
Referring a client to San Jose Behavioral Health is an efficient and stress-free process that begins with a simple phone call. Professionals may call (669) 234-5959 to begin the referral process. This number will reach a member of our intake staff, who will complete an initial phone screening to assess specific treatment needs and recommend the appropriate level of care. Intake assessments are available 24 hours a day, seven days a week.
Once a referred patient has been admitted for treatment, we will be happy to coordinate care with referral sources who had been working with the patient prior to his or her admission to San Jose Behavioral Health. With the patient’s permission, we will provide regular updates to referents throughout the treatment process. These updates can be delivered in the manner and frequency that best serves the needs of the patient and is most useful to the referring professional
Of course, at San Jose Behavioral Health we recognize that a patient’s time in our program is just one phase of his or her overall treatment and recovery experience, and we place a premium on ensuring that our clients have the comprehensive support that they need to maintain and build upon those successes once they have completed their time with us. Effective communication between our program and the professionals who work with our patients before and after their time with us can be an essential component of this comprehensive support system, and we will be sure to refer the patient back to the referring professional when clinically appropriate and in the patient’s best interests.
